*For the concert, all performances are 30 minutes . All performers must dress in costumes related to the theme of the concert which showcases your country's royal institution (ancient or present).

Yes, I have read and agree to the Terms and Conditions. Terms and Conditions, I, and members of my group would like to participate in The Gathering XIV concert: a musical showcase of traditional royal institutions around the world event organized by New York African Chorus Ensemble Inc and its partners, and scheduled for Saturday, December 7, 2019, at P.S. / I.S. 210 Twenty- First Century Academy, for community Leadership. 501 W 152nd Street New York, NY 10031. I understand, acknowledge and agree to the following: 1. There will be no binding relationship between me, my group and New York African Chorus Ensemble Inc. 2. I must submit all required documents on the date requested. 3. I have no physical limitations that might keep me from performing the services associated with the Event. 4. In consideration for being allowed to participate in the Event, I hereby personally assume all risks in connection with the Event for any harm, injury, or damage that may befall me as a volunteer or participant, including all risks connected therewith, whether foreseen or unforeseen. I further save and hold harmless New York African Chorus Ensemble Inc., its partners, the City of New York and their directors, officers, employees, agents or representatives (“Released Parties”) from any claim or lawsuit for personal injury, property damage, or wrongful death, by me, my family, estate, heirs, or assigns, arising out of participation in the Event, including both claims arising during the Event and after I complete my services for the Event. I grant and convey to New York African Chorus Ensemble Inc. all right, title, and interest in any and all photographic images and video or audio recordings of me or made by me during the Event or in rehearsals and other activities prior to the Event. I HEREBY EXEMPT AND RELEASE ALL “RELEASED PARTIES” AS DEFINED ABOVE, FROM ALL LIABILITY OR RESPONSIBILITY WHATSOEVER FOR PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH, HOWEVER CAUSED, INCLUDING NEGLIGENCE OF THE RELEASED PARTIES, WHETHER ACTIVE OR PASSIVE, ARISING OUT OF MY VOLUNTEER SERVICES FOR THE EVENT. I AM AT LEAST EIGHTEEN YEARS OF AGE AND AM COMPETENT TO SIGN THIS RELEASE AND WAIVER OF LIABILITY ON THE BEHALF OF MYSELF AND MEMBERS OF MY GROUP. I HAVE CAREFULLY READ THE TERMS AND CONDITIONS OF THIS RELEASE AND WAIVER OF LIABILITY AND UNDERSTAND AND AGREE TO THEM.
